Kick-starting Connect-Home Collaborative

Lutheran Services in America had the pleasure of hosting 4 of our members and partners from the University of North Carolina at Chapel Hill at our office for the kick-start of our 18-month Connect-Home Collaborative. We were joined by nursing home staffers and leaders from Samaritas, Lutheran Social Services of New Jersey, National Lutheran Communities and Services and Lutheran Services of the Carolinas. 

The Connect-Home Collaborative aims to implement transitional care services for our member organizations with nursing homes. The collaboration is designed to prepare skilled nursing facility (SNF) patients and their caregivers on safely transitioning from SNFs to home-care without many complications. The solution has been successfully tested by Lutheran Services Carolinas and has officially expanded to the three additional members.

Dr. Mark Toles, an assistant professor with expertise in services to promote safe transition in care for older adults, introduced the members to data-driven research on skilled nursing facilities that he has gathered over the past 10 years. The four Lutheran Services in America members joined in on an interactive Transition Plan of Care template to map out current discharge planning processes and changes that they will need to consider as they roll-out Connect Home.

The day was full of enlightening dialogue, and we are all excited about this great opportunity for us all to learn from each other while continuing to serve older adults.
